2012 Prevention and Public Health Funds: Empowering Older Adults and Adults with Disabilities through Chronic Disease Self-Management Education Programs

Purpose and Background

The Empowering Older Adults and Adults with Disabilities through Chronic Disease Self-Management Education Programs (CDSME) funding opportunity is financed through the 2012 Affordable Care Act Prevention and Public Health Fund (PPHF).

  • Evidence-based, self-management programs provide older adults and adults with disabilities with education and tools to help them better manage chronic conditions such as diabetes, heart disease, arthritis, HIV/AIDS and depression.
  • The overall purpose of this initiative is to help ensure that these programs are embedded into the nation’s health and long term services and supports systems.

This effort will help preserve and expand the prevention program distribution and delivery systems that were developed through the Recovery Act Communities Putting Prevention to Work: Chronic Disease Self-Management Program grants.

  • These prior Recovery Act two-year grants, which began March, 2010, were intended to deliver evidence-based self-management programs to 50,000 older adults with chronic diseases and to build statewide distribution and delivery systems that increase access to these programs.
  • As of August 28, 2012, the 47 Recovery Act grantees had hosted over 10,000 CDSMP workshops at nearly 6,300 unique implementation sites.
  • These workshops reached over 111,000 participants, with over 83,000 completing at least four of six sessions-- far exceeding the original grant goals.

The new PPHF CDSME cooperative agreements are designed to achieve the following two major goals:

  • Goal 1: Significantly increase the number of older and/or disabled adults who complete evidence-based CDSME programs to maintain or improve their health status.
  • Goal 2: Strengthen and expand integrated, sustainable service systems within States to provide evidence-based CDSME programs.

To help embed the programs into existing long-term care and health systems, State Units on Aging and State Public Health departments will work together to develop sustainable service systems.

  • Six states selected their State Public Health Department to be the lead and 16 states have a State Unit on Aging lead.
  • These agencies have proposed to work with other State agencies and with local agencies on aging, health departments, Centers for Independent Living, minority organizations and other local partners to expand their services systems.
  • To help build their sustainable infrastructures, grantees will utilize diverse strategies including:
    • embedding programs within Affordable Care Act initiatives such as care transitions and medical homes,
    • partnering with Medicaid and other health insurance providers,
    • pursuing accreditation and Medicare reimbursement for Diabetes Self-Management Training,
    • collaborating with Federally Qualified Health Centers, Veterans Administration Medical Centers and other healthcare organizations, and
    • teaming up with non-traditional partners such as the State Department of Corrections and State and Local mental health agencies.

Applicants for this funding opportunity could have proposed either the Stanford University Chronic Disease Self-Management Program or another type of CDSME program that meets AoA’s highest-level evidence-based criteria.

Funding History

  • The U.S. Administration on Aging, part of the Administration for Community Living, awarded over $8.58 million in cooperative agreements to 22 states on September 20, 2012.
  • Depending on the availability of continued funding for years two and three, the grants are for a three-year project period ending August 31, 2015.
