Patient Concerns

You have the right to present concerns pertaining to patient care and safety at all VISN 18 Health Care System locations including Outpatient Clinics. These concerns should be directed to the Manager of the Service or Department that is providing care, the patient advocate or the facility director. If you are unsure of whom to contact, any employee can direct you to the appropriate supervisor.

Should it appear that your concerns can’t be resolved at the local facility, you should contact VISN 18 at:

Network Office VISN 18 (10N18),
6950 E. Williams Field Rd.,
Mesa, AZ 85212

Phone: (602) 222-2681

If at any time you feel that hospital management has not addressed or resolved the concern, you may contact the Joint Commission at the following address:

Joint Commission
Office of Quality Monitoring
One Renaissance Boulevard
Oakbrook Terrace, IL 60181

Or by calling (800) 994-6610 or on line at  http://jcwebnoc.jcaho.org/QMSInternet/IncidentEntry.aspx

The Joint Commission standards deal with organizational quality of care issues and the safety of the environment in which care is provided.
