Adding Extra Pages to Your Passport

If you have fewer than four Visa pages remaining in your passport book  you can have extra visa pages added to your passport book.

Anyone seeking to get extra pages to a U.S. passport must apply in person at the American Citizen Services office at Embassy Warsaw, Consulate General Krakow, or the Consular Agency in Poznan.

Please bring your current passport with the completed form DS-4085.  (The link for the passport application appears below.)

Fees and Forms

The passport fees below are the most current as of July 13, 2010.

  • For all applicants a fee of 82USD.
