Requests for Personal Records

Any individual may request access to records about himself or herself from an OGE Governmentwide system of records. To do so, an individual must submit a written request, which includes the words “Privacy Act Request” on both the envelope and at the top of the request letter. A request should contain a specific reference to the OGE system of records from which access to the records is sought, the requester’s full name (including maiden name, if pertinent), and a statement whether the requester wants a copy of the record or wants to examine the record in person.

OGE maintains two major Governmentwide systems of records and five Internal systems of records.

The Governmentwide systems of records are:  

  • Executive Branch Personnel Public Financial Disclosure Reports and Other Name-Retrieved Ethics Program Records (OGE/GOVT–1) 
  • Executive Branch Confidential Financial Disclosure Reports (OGE/GOVT–2).

The five Internal systems of records are:  

  • Pay, Leave and Travel Records (OGE/INTERNAL–1)
  • Telephone Call Detail Records (OGE/INTERNAL–2)
  • Grievance Records (OGE/INTERNAL–3)
  • Computer Systems Activity and Access Records (OGE/INTERNAL–4)
  • Employee Locator and Emergency Notification Records (OGE/INTERNAL–5).

A requester may provide his or her dates of employment if relevant to the inquiry.
NOTE: A requester seeking access to a SF/OGE Form 278 or OGE Form 450 may file with OGE a Privacy Act request or an OGE 201 Public Access Form.

For records filed with a Designated Agency Ethics Official (“DAEO”) or the head of a department or agency, a request should be sent to the DAEO at the department or agency concerned.
