Community-Based Outpatient Clinic
The Guayama Community Based Outpatient Clinic is a Satellite of the Ponce VA Outpatient Clinic and of the San Juan VA Medical Center, and was established in 2003 to improve access to Primary Care services for veterans in Southeast Puerto Rico. We currently provide a variety of services:
- Primary Care
- Preventive Health Care
- Care of Chronic Conditions
- EKG
- Phlebotomy/Laboratory Testing
- Prescriptions: Routine prescriptions are processed through the main facility, mail or My HealtheVet.
The Clinic also provides some services with visiting staff from the Ponce Outpatient Clinic and the San Juan VA Medical Center. Services that are not available in the Community Based Clinic can be obtained via referral to local health care specialists or to the VA Medical Center in San Juan.
Please note that Community Based Clinics do not have emergency rooms, and are not the facilities of choice in case of life threatening emergencies. In such cases patients are referred to the Hospital Cristo Redentor emergency room.
Registration and Enrollment Information
Patients not previously registered may do so at the Ponce VA Outpatient Clinic, where initial medical evaluation with laboratory or x-ray tests will be performed prior to enrollment at the Guayama CBOC.
Driving Directions
From the North starting on EXPRESO LAS AMERICAS (Plaza Las Americas Shopping Center) heading South. Continue on A-18 S. Merge with A-52 S (Portions toll) go 37.5 miles. Take LEFT exit #60/GUAYAMA onto A-53 E (Toll applies) go 7.6 mi, continue on A-54 E (Toll applies) go 2.1. Turn LEFT on C Paseo del Pueblo go 0.2 miles. Arrive at Guayama CBOC on the LEFT.
