Special Requirements for Minor Children under Age 16

It is mandatory to include on the online appointment request the names of all non-applicants (parents and guardians of a minor) who plan to accompany the applicant to the Consular section; only those whose names are on the appointment list will be granted entry.

Both parents, along with each minor, must be physically present in the Consular Section to sign a minor's passport application. If one parent is not physically present, then notarized form DS-3053 should accompany the application. If the missing parent is not in the U.S. then the form can be notarized at the nearest U.S. Consulate. A certified true copy of the missing parent’s passport or photo ID should accompany form DS-3053. 

MINOR U.S. CITIZENS (under Age 16) must submit:

• Form DS-11
• Last U.S. passport
• 1 photo (2" x 2" inches or 5" x 5" cm) taken within the past 6 months (See our photo specifications page)
• Parents’ passports
• Fee of $105, payable in U.S. dollars or the Georgian Lari equivalent (accepted cash only)
• Progressive photos of minor’s development: Photos to demonstrate the child in the old passport is the same child presented at the time of interview.  Generally, one photo from each year since the prior passport was issued is sufficient.
• Birth certificate or Consular Report of Birth Abroad (original for first time passport applicant; copy for passport renewal)
• If the parents have authorized a legal guardian to apply for a passport on behalf of their child, the legal guardian must submit a passport or photo ID and a notarized power of attorney in English that specifically authorizes him/her to apply for the passport on behalf of the child.
• If one or both parents are not physically present, then notarized forms DS-3053 should accompany the application. If the missing parent is not in the U.S. then the form should be notarized at the nearest U.S. Consulate.  A certified true copy of the missing parent’s passport or photo ID should accompany form DS-3053. 
  

Processing Time

You should be prepared to allow from 10-14 business days to receive a new passport.
