Family Health

·        What services are provided by the Family Health Clinic?

 

The Family Health Clinic provides Primary Health Care Management Services for enrolled beneficiaries. Services offered in the clinic are: acute, routine, wellness, and established appointments, to include Physical Health Assessment (PHA) physicals; school and sports physicals; and follow-up appointments; prescription renewals (refills); pregnancy tests and blood pressure checks.

 

·        What are the sick call hours for active duty?

 

Sick call is by appointment only.  Appointments are made by calling the Hampton Roads Appointment Center at 1-866-645-4584. 

 

·        What is the process for pregnancy tests?

Pregnancy tests are offered from 0800-1500 Monday through Friday for enrolled beneficiaries. Test results are generally ready in one hour or patients may complete consent to receive results via telephone.

·        What is the process for the Shaving Clinic?

The Shaving Clinic is open on Wednesdays from 1300-1540.  Referrals are not necessary but patients are seen by appointment only.  Appointments are made by calling 314-7500 ext. 44024.

·        What is the process for the Wart Clinic?

 

The Wart Clinic is open on Wednesdays from 1300-1540.  Referrals are not necessary but patients are seen by appointment only.  Appointments are made by calling 314-7500 ext. 44024.

 

·        What is the process for medication refills and referrals to the Family Health Clinic?

For medication refills for Family Health Prime enrollees only call 878-7500 ext. 44000 or 314-7500 ext. 44000.  For general referral information call 314-7500 prompt 3 and then prompt 1 again. 

 

Physical Exam

·   When are the DODMERB Physicals conducted?

DODMERB Physicals for students are conducted from 0700-0800 on Thursdays by appointment only.  For questions call 314-7500 ext. 44024

·   What is the process for Active Duty Physicals?

Physicals are conducted in two parts. Part (1) appointments are Monday and Wednesday, and walk-ins on Tuesdays and Fridays. Part (1) consists of paperwork for physicals, and Part (2) consists of an actual physical exam by the healthcare provider. Part (2) appointments are made by the clinic when Part (1) is completed

 

Aviation Clinic

·        Where do I go for my Aviation Physical?

The Aviation Clinic is located at Troop Medical Clinic I (TMC 1) Bldg 664 on Washington Blvd.  Hours of operation are 0700-1200 and 1300-1600 daily.  The clinic is closed on Training Holidays. 

 ·        What services are provided in the clinic?

Part I & Part II Physicals for Flight and Diving. Provide medical care for assigned Aviators/Crew and Divers.  Part I of the Physical is walk-in Monday-Friday at 0730. Part II will be scheduled after Part I completed.  For questions or appointments contact the clinic at 878-1686.

 

Pediatric Clinic

·        What can be covered in a Telephone Consult?

 Telephone consults/telephone messages are used for referral questions, medication refills, and to leave a message if patient and/or parent needs to speak with a provider. Please allow 48 hours for referral and medication refill requests to be processed.

·        What is the process for Well Baby Immunizations?

Well baby immunizations will be given per the Centers for Disease Control recommendations, in the Pediatric Clinic for those that have Well Baby appointments only. Other immunizations will be given in the Immunization Clinic.

 

Exceptional Family Member Program (EFMP)

·   Is EFMP enrollment mandatory for family members with special needs?

EFMP enrollment is mandatory and required immediately upon identification of a family member with chronic health conditions and/or special needs. Dependents must be enrolled in your sponsor’s service-specific EFMP to receive benefits from TRICARE Extended Care Health Options (ECHO).

·   Is Overseas Screening mandatory for my family members?

Yes. This is done through the EFMP office for family members located in the Disease Management Clinic.

·   What are the enrollment requirements for the Army Couples Program?

If you are a member of the Army Couples Program and a family member qualifies for the program, both persons must enroll in the EFMP. This will ensure that both assignment managers consider your family’s special needs.

·   What forms are used for the EFMP Program and where may I obtain them?

The forms used to enroll, update or dis-enroll from EFMP are DD Form 2792-Medical Summary, DD From 2792-1 Special Education/Early Intervention Summary. These forms are available at AKO.

 

Internal Medicine

·   What is the refill policy/process for Internal Medicine?

Patients need to have been seen by a healthcare provider within the last six months. Patients are asked to bring the empty containers/pill bottles into the clinic for review and medication reconciliation. The provider will request an appointment, should there be any medical concerns or questions; otherwise the prescription refill will be ready for pick up the following day at the Main Pharmacy.

·   When are EKGs done?

EKGs are done on a walk-in basis Monday through Friday from 0800-1130 and 1300-1600.

·   What is the process for Respiratory Therapy?

Referrals and appointments are required for Pulmonary Function Testing. Appointments can be made by calling 314-7945

 

Dermatology

·   Does the Dermatology Clinic remove warts?

No. The Wart Clinic is located in the Family Health Clinic on the third floor. If the provider feels you need to seen by the Dermatologist a referral will be entered for you at that time. The Wart Clinic is scheduled on Wednesdays from 1300-1540 hours by appointment only.

 

Immunization Clinic

·   Do you give immunizations from civilian providers?

Yes. These patients are required to bring in a hand written prescription.

·   Are prescriptions required for certain shots?

Yes. Prescriptions are required for B12, Testosterone, Zostavax (Shingles) and Gardasil (HPV) shots.

·   Are Tuberculosis skin tests (PPD) administered every day?

No. PPDs are administered every day except Thursday.

Please Note: All female patient age 12 and above are required to have a pregnancy test prior to the administration of a live virus.

 

Allergy Clinic

·   Are civilian allergy serums accepted at the clinic?

No. There are strict standards regarding internal control and physical integrity of allergy serum accountability we must adhere to.

 

Laboratory

·   Can I pick up lab results at the Lab?

No. The laboratory is not authorized to release lab results to patients; they are only authorized to release results to the ordering provider. During the appointment your provider may explain that he/she will contact you only if the test result is abnormal. If you would like the result, please contact the clinic. If you are a patient with a prescription for a lab test from a civilian provider, the results will be sent directly to the ordering provider. If the civilian provider has not received the results, you can contact the Release of Information office for assistance at 314-7868.

 

Nutrition Care

·   I am Active Duty. How do I sign up for the Weight Management Program?

Active duty service members desiring to participate in the weight management program can call 314-7759.

 

Women’s Health Clinic

·   Do I need a referral for my annual exam?

No referral is needed for Nurse Practitioner wellness appointments, i.e., annual exams, PAP smears, and/or evaluation for birth control. A referral is required to see a GYN Physician.

·   How do I get a pregnancy test?

Pregnancy tests are offered in the Family Health Clinic from 0800-1500 Monday through Friday for enrolled beneficiaries. Results are generally ready in an hour or patients may complete consent to receive results over the phone.

·   How do I get my birth control pills refilled?

Walk-in hours for patients who are currently taking birth control medication are from 0730-1100 and 1300-1500. To obtain a refill the patient must have documentation of a current Pap test (within one year) and have no existing medical problems. Your medical record will be reviewed, vital signs will be taken, and the prescription will be available for pick up at the Main Pharmacy the following business day.

·   How do I get my Depo Provera shot?

Shots are administered by appointment only. To schedule an appointment for your shot please call 314-4040.  Your medical record will be reviewed before we can administer the injection. Medical record must contain evidence of; Pap test; current order for Depo Provera; and date of last injection to include dose and injections site. Depo Provera orders written by healthcare Providers outside of McDonald Army Health Center must be reviewed and rewritten by a provider privileged within our Women’s Health Clinic. If the order needs to be reviewed and rewritten please call 1-866-645-4584 to schedule an appointment.

·        How do I get my Lupron injections?

Lupron injections are administered by appointment only.  To schedule an appointment for your injection please call 314-4040. Your medical record must be reviewed before we can administer the Lupron injections. The medical record must contain the following documentation: date of last injection; dosage given; and injection site. The Lupron order must be written by a provider privileged within our health facility.

 

Patient Administration Division

·        How do I get a copy of my medical records or how do I get my medical records transferred here from another facility?

Copies of medical records, transferring of medical records, and requests for medical records are accomplished through the Release of Information office in Room M27. Patients must present in person for these services.

·   Where do I take my Profile and Line of Duty (LOD) documents?

To have your Profile and/or Line of Duty documents processed, please take them to the Profile/LOD Office in room M33.

 

Preventive Medicine Services

·   I am active duty. How do I get an HIV test?

Active duty HIV testing is done at Community Health in Bldg. 2792A on Tuesdays and Thursdays. Personnel are to report no later than 0815.  There is a required briefing that starts at 0830 and labs are drawn following the briefing. There is no testing the second Tuesday of the month. ·   Where can I get a Self-Care card for over the counter medications?

Self-Care classes are given at Community Health in Bldg. 2792A every Monday at 1030 for TRICARE Prime and TRICARE Plus assigned beneficiaries only. For additional information call 757-878-4755.
