Discharge Procedures

 

Sacramento VA Medical Center

Early during your stay, your health care team will talk to you about your care needs after hospitalization, your anticipated discharge date, and how to receive your follow-up care.

If supplies are needed at the time of discharge, the discharge nurse will review them with you. If any medications are prescribed, they can be picked up at the outpatient pharmacy as you leave the hospital.

If you are on a special diet, a dietitian will counsel you and your family on how to follow your diet at home.

For questions about transportation home, please speak with the unit social worker. And, before you leave the unit where you are a patient, please make contact with the nursing staff or your doctor.

Martinez Center for Rehabilitation & Extended Care

Prior to admission, or early during your stay, your interdisciplinary treatment team will talk to you about your goals for your inpatient stay and they will discuss your anticipated discharge date. You will be discharged when the goals of your plan of care have been met or it is determined that you have received the maximum benefit from your stay.

Before discharge, you will be given instructions from the pharmacist, nurse pracitioner or attending physician, nurse, and social worker regarding your continued care. Also, if needed, you will receive dates and times for follow-up appiontments.

If you are on a special diet during your stay, a dietician will counsel you and your family on how to follow your diet at home.

Lost and Found

If you have lost any personal items during your stay, tell a member of your health care team. You may also call our Lost and Found Office at the Sacramento VA Medical Center at (916) 843-5401 or the VA Martinez campus at (925) 372-2345.

Patient Satisfaction Surveys

You may be contacted for feedback (by mail or phone) about your stay shortly after your discharge to complete a patient satisfaction survey. Your comments will help us to continue to meet our goal of providing high quality health care services to our patients. It is important that we have your correct address and phone number at time of discharge, especially if you will not be going to your own home.
