New Student

It is a requirement that New Sure Start students must be 4 years old by September 1 and new Kindergarten students must be 5 years old by September 1. In order for your child to be OFFICIALLY ENROLLED in school, we MUST HAVE ALL the following documents two days in advance before the start of school.

For Military Sponsor:

• Copy of Sponsor's current orders
• Copy of Family Entry Approval or Letter of Command Sponsorship

For DoD Civilian Sponsor:

• Copy of DD Form 1614
• Copy of Letter of Emplyoment

For ALL Sponsors:

• Copy of student's Passport and Birth Certificate
• Current Immunization Records
• All hand carried previous school records (if applicable)
• Special Education File from previous school (if applicable)
• Space Available Notification - DSPAM Form 2030.1 (if applicable)

Returning Student

Sponsor or spouse must fill out the following documents:

• Sponsor/Pupil Registration (DS Form 600) (pdf, 66 KB)
• Update for School Health Record DoDEA Form 2942-0-M-F2 (pdf, 59 KB)
• Present an updated copy of sponsor's orders if needed. This is only necessary if the sponsor has received an extension of the original orders, or if the sponsor has been assigned to a different command within the commuting area.
