OEF/OIF/OND Care Management

As a recently returned combat Veteran, we are here to serve you.  The Operation Enduring Freedom/Operation Iraqi Freedom/Operation New Dawn (OEF/OIF/OND) Care Management Team provides comprehensive knowledge and care coordination to assist you in maximizing your services and benefits. As an OEF/OIF/OND Veteran, you are eligible for five years of free healthcare for conditions potentially related to combat.

OEF/OIF/OND Seamless Transition Clinic

Seamless Transition is a one-time clinic appointment for combat veterans.  When you attend this clinic, you will meet with three providers that will screen for post-deployment needs.

  • Primary Care Provider: to discuss medical concerns/needs;
  • Behavioral Health Provider: to discuss mental health concerns/needs;
  • OEF/OIF/OND Social Worker: to discuss benefits, services, and case management needs.
