Title: State Partnership Grant Program to Improve Minority Health
Deadline: August 13, 2010
Awards: $6,450,000; $125,000- $140,000 per state/territory
Project Period: 3 years, September 1, 2010 - August 31, 2013
Purpose: To demonstrate the effectiveness of strategic partnerships to improve the status of minority populations and eliminate disparities in at least one of the following health topics: access to healthcare, asthma, cancer, cardiovascular disease/stroke, immunizations, diabetes, HIV/AIDS, infant mortality/LBW, mental health and/or obesity.
Eligibility: The applicant must be an established state or territorial office of minority health at the time of application, and must provide a letter or commitment for the proposed project from an authorizing state or territorial official.
