First, please read: Applying for a U.S. Passport from Outside the United States. Also, do not print applications double sided, one sided only please.

If this is a first time passport application for a child born outside the United States please go to Consular Reports of Birth Abroad.

I. Under the age of 16 years

1. Applicants must appear in person accompanied by parents;
2. Present proof of U.S. citizenship;
3. Provide parental identification;
4. Present notarized form Parental Permission - DS-3053 (fillable) if one parent is unable to be present at the time of application;
5. Complete form DS-11, (please disregard instructions on form and do not sign until requested to do so by the consular officer);
6. Pay the $105 application fee;
7. Submit one (1) U.S. passport photo.
8. If the application is approved you will receive a 5-year passport.
9. Once you have completed the above steps, please book your appointment on-line.

II. Age 16 years and Over 

1. Applicant must appear in person;

2. Present proof of U.S. citizenship;

3. Present proof of identity (i.e. driver's license);

4. Complete form DS-11 (please disregard instructions on form and do not sign until requested to do so by the consular officer);

5. Pay the $135 application fee;

6. Submit one (1) U.S. passport photo;

7. If the application is approved you will receive a 10-year passport.
