Format for Federal Agency Proposals Submitted to the Office of Science
What to Submit
- Cover Page Format
- Name and address of Federal agency submitting the proposal
- Proposal submission date
- Title of the Project
- Proposed starting date and estimated period of performance
- Name of DOE/Office of Science program contact (if known)
- Principal Investigator (Name, telephone number, fax, and e-mail address)
- Signature of the principal investigator
- Agency Business Contact or Program Official (Name, telephone number, fax, and e-mail address)
- Signature of Agency Business Contact or Program Official
- The cover page information may be placed on plain paper. No form is required
- Budget Information
- A cost estimate for the proposed effort sufficiently detailed by each element of cost
- The budget may be placed on plain paper. No form is required
- Technical Information
- A 200-word abstract of the proposed work
- The body of the proposal should contain a statement of the work to be performed with sufficient technical detail to permit a meaningful evaluation.
- Describe the available facilities and major items of equipment, if applicable
- Names of key project personnel including the principal investigator or project leader along with brief biographical information on each
- If submitting a renewal proposal, the submission should include a progress report that covers significant results obtained since the inception of the agreement
