Soldier Medical Readiness Clinic

Hours: 0700 - 1600 Monday, Tuesday, Thursday, and Friday.  CLOSED every 2nd and 3rd Thursday at 1200 for mandatory training.

 

Clinic is reserved every Wednesday for deployment screenings!

 

CLOSED  from 1200- 1300 for lunch

Medical In-Processing is conducted on Monday, Tuesday, and Friday at 0830.  

 

The Solider Readiness Clinic (SRC) conducts Pre-deployment Assessments, Post-deployment Assessments, and Post-deployment re-evaluation assessments, and Medical In-Processing.

 

MISSION

 

The mission of the SRC is to ensure all Soldiers comply with deployment standards as outlined in Personal Policy Guidance, MOD 11 and Personal Policy Guidance, Tab A: Amplification Of The Minimal Standards of Fitness for Deployment To The CENTCOM AOR. 

 

PURPOSE

 

To ensure that all Soldiers traveling to Theater are medically and psychologically fit for deployment. To ensure that all Soldiers receive quality assessments upon return from Theater to identify any medical or psychological conditions and provide evaluation and treatment services.

 

Requirements for Pre-Deployment Screening/ Soldier Readiness Processing (SRP):

 

1.  Current Periodic Health Assessment (PHA).  Must be within 1 year of Pre-deployment screening.  PHA consist of two parts:  Part I:  Hearing, vision, computer survey, lab work & EKG (if 40 years and older).  Part II: Assessment with a Medical Provider.  Appointments are scheduled in the Active Duty Clinic.

 

2.  Compliance with Women's Readiness Guidelines:  Women under 21 years of age (i.e., up to age 20 years and 364 days) are not required to have a pap test.  Women 21 years of age and older are screened every 2 years, unless instructed otherwise by physician.  Women age 30 and aoldera re screened every 3 years unless otherwise determined by other factors.  Chlamydia and breast cancer screening will be in accordance with (IAW) AR 40-501 at their Periodic Health Assessment (PHA).  Mammography is currently recommended biennially ( every 2 years) for women aged 50 to 74 years old.

 

3.  Automated Neurological Assessment (ANAM) test within 12 months of deployment.  Appointments for ANAM testing can be made by calling 765-7646.

 

4.  Complete Pre-deployment Survey (DA Form 2795) through AKO no sooner than 5 days prior to SRP

 

  The above requirements must be complete prior to the medical Pre-deployment screening date.

 

 

During the SRP:  Soldiers will receive all required vaccinations, and provide blood samples, and urine samples as needed.  All Soldiers will be assessed by a Medical Provider and all information will be updated in MEDPROS during the processing. 

 

 

Advice:

 

1.  Do not schedule SRP backed up to block leave, remote training, or deployment because Soldier's with a identified medical or psychological condition will require further evaluation and clearance. 

 

2.   All Soldier's must have a Automated Neurological Assessment (ANAM) test within 12 months of deployment. ANAM appointment

 

3.  Soldiers must know their AKO account number to complete DD 2795 and sign the form with the medical provider.

 

4.  SRP team does not control the internet function or speed of MEDPROS(which goes down at times).

 

5.  Do not schedule appointments or other commitments on the "Medical SRP" day.

 

 

Requirements for Post-Deployment Health Screening:

 

1.  Complete Post-deployment Survey (DA Form 2796) through AKO no sooner than 5 day prior to medical screening.

 

Advice:

 

1.  Soldiers must know their AKO account number to complete DA 2796.

 

2.  SRP team does not control the internet function or speed of MEDPROS(which goes down at times). 

 

 

Requirements for Post-Deployment Health Re-Assessment:

 

1.  Call the Soldier Readiness Clinic at 765-7646 to schedule an appointment.

 

2.  Complete DD 2900 through AKO deployment website prior to appointment.
