Submitting Follow-Up

The CDC and FDA require additional information on selected VAERS reports for the public health purpose of helping to ensure the safety of U.S.-licensed vaccines. You or your health care provider may be contacted for follow-up information by VAERS staff after your report is received. These selected reports are followed up by a team of health care professionals to obtain additional information (such as medical records and autopsy reports) to provide as complete a picture of the case as possible. All records sent to VAERS are kept confidential as required by law. The patient's consent is not required to release the medical records to VAERS. You are welcome to use the VAERS Auto Upload online tool to submit additional information.

About the Auto Upload tool

  • A one-time-use key is required in order to access the tool.

    • Reporters may request a key by contacting VAERS via e-mail at info@vaers.org. Be sure to have your VAERS ID or E-number for reference.

    • Individuals assisting us in our follow-up efforts may also receive a key by VAERS staff.

  • The key is sent via e-mail.

  • The tool uses the key as well as the patient's name and date of birth to authenticate a session.

  • The key is case sensitive.

  • You may upload up to 3 documents per session. You may contact VAERS to request other keys.

  • The tool accepts commonly used document formats (such as .doc, .pdf, .jpeg and etc.).

  • The combined document size limit is 100MB per session.

  • Individuals receive a confirmation at the end of submission.
