OEF/OIF/OND Veteran Transition Center and Care Management Team

The goal of our OEF/OIF/OND Veteran’s Transition Center and Care Management Team is to ensure returning combat Veterans experience a smooth transition from the war front to the home front and to help you maximize your services and benefits.

Before you do anything, please call Veterans Transitions Center at 702-224-6029 and set up an appointment to get connected with our Care Management Team.  We can help you get through the questions and obsticales you may otherwise experience.  If you are a post-9/11 combat Veteran coming in to enroll into the VA for health care, see us first at the Veterans Transition Center at the new VA Medical Center, 6900 North Pecos Road, North Las Vegas, NV 89086, in Suite 1C157.  Some of the common transition needs we can assist with include:

  • Applying for VA, other government, and community benefits
  • Care coordination & advocacy
  • Resources for marriage, family, and spirituality concerns
  • Aid with concerns about drinking or drug use

  • Employment resources
  • Assistance with other issues as needed
