Our Returning Service Members program (Seamless Transition) includes care coordination and support services. We are your first stop in establishing your VA Health Care, and we can help you access and manage your post-deployment services. This program is specifically for combat veterans who served on active duty in a theater of combat operations after November 11, 1998, and should have received an honorable discharge.
This program supports you as you're transitioning from active duty military to civilian life. We help you receive the health care services you may want or need, and we provide you and your family with the following FREE support and service options:
- Readjustment to life after military service;
- Individualized care, specific to your unique post-combat readjustment needs;
- Professional and confidential individual, marital/couple, family and group counseling;
- Assistance scheduling and accessing any needed VA Medical Center services;
- Contact information on VA services near your home;
- Information on VA benefits for which you may be eligible;
- Recreational opportunities for you and your family;
- Personal assistance finding your way through the VA system;
- Watch this video as Combat Veterans describe their experience with our services.
