The 386th Air Expeditionary Wing has a diverse mission which canvases the U.S. Central Command area of responsibility. The 386th AEW provides airlift support for Operation Enduring Freedom and the Horn of Africa. The wing is comprised of the 386th Expeditionary Maintenance, Mission Support, Medical and Operations Groups and the 387th Air Expeditionary Group.

tab386th Expeditionary Maintenance Group 
The 386th EMXG supports the wing mission by providing C-130 combat airlift and EC-130 electronic attack maintenance support to the theater commander. The Airmen of the 386th EMXG provide common intermediate maintenance support to tenant C-17 detachment and organizational/intermediate maintenance support for transient aircraft. They also provide command, control, and operational guidance for two assigned Maintenance Squadrons and associated support functions. The group directs maintenance operations in an expeditionary tactical airlift/electronic attack environment in the USCENTCOM AOR, while providing Maintenance Recovery Teams (MRTs) and required equipment to support, repair, and recover Non-Mission Capable theater assets when and where needed.
tab386th Expeditionary Medical Group  
Supports the wing mission by ensuring a healthy and fit force and by providing combat medical support for America's heroes and coalition partners. Assists in the maintenance of wing combat capability by providing local emergent and routine healthcare, coordinating local specialty medical referrals and aeromedical evacuation out of the AOR when necessary. Additionally coordinates and supports movement of casualties throughout the AOR, provides combat stress management, preventive medicine support, dental and force health protection for DoD, US and coalition forces throughout Southwest Asia.
tab386th Expeditionary Operations Group 
Delivers Total Force tactical airlift and intelligence/surveillance/reconnaissance for combat ops, combat support ops, and humanitarian relief ops throughout the USCENTCOM AOR. Operates C-130s & MQ-1Bs with crews from 4 MAJCOMs in the full spectrum of joint/coalition engagement operations. Oversees 4 flying detachments to include C-17/AH-64/C-12/UC-35 aircraft. Supports airfield operations w/over 50,000 coalition departures/arrivals a year.
tab386th Expeditionary Mission Support Group 
The 386th EMSG provides agile combat support to the air expeditionary wing and base operating support-integration for the 387th Air Expeditionary Group. It is responsible for base defense, internal security, fire protection, civil engineering, personnel management and accountability, communications, contracting, supply, transportation, fuels, aerial port, lodging, dining and recreational services. The group directs O&M expenditures in excess of $59M annually and is the principal wing liaison with coalition partners and senior military counterparts for all base service support and logistics issues.
tab387th Air Expeditionary Group  
387th AEG provides base operating support-integration for Coalition forces and civilian contractors at the United States Air Force busiest Aerial Port of Debarkation in the world. The Airmen of the 387th provide security, anti-terrorism and force protection, civil engineering, emergency management, personnel support, vehicle maintenance/fleet management and base supply in support at the primary intra-theater airlift hub for all Joint and Coalition operations going into and out of Iraq and Afghanistan.
