Registration Information

Lanham ES PCS Checklist

To ensure a smooth transition for your school age students, remember to bring the following documents with you in person when you PCS to NAF Atsugi, Japan.

  • Sponsor's Orders
  • Dependent Entry Documentation
  • Immunization record
  • Passport
  • Copy/official transcript
  • Copy of IEP if applicable
  • Copy of withdrawal grade with grading system from previous school
  • Copy of standardized test score if applicable
  • Copy of current school schedule (HS students)

Eligibility for Enrollment
There are specific laws and policies that determine if your children are eligible to enroll in DoDEA schools. Likewise, there are a number of different categories of enrollment they may fall in to which vary between DoDDS and DDESS schools. In DoDEA Pacific we have both types of schools. Three of our four districts are DoDDS (Japan, Okinawa, Korea) while one of them is a DDESS school district (Guam).

IMPORTANT:
You must still complete the registration process when you arrive.

New Students Returning Students
Sponsor must comoplete and submit the following
documents:

* Sponsor/Pupil Registration (DSPA Form 2030.2)
* Sponsors must sign the DS 600. The sponsor's signature is required even if the spouse has a Power of Attorney due to regulations regarding the allocation of funds
* If the sponsor is not in the country, temporary arrangements may be made to allow the child to attend school
* School Health Record (DoDDS Form 120)
* Please include any special health concerns that may be required for your child's safety and well being

At the time of registration, Sponsor must present the following:
* Copy of military sponsor's orders or Letter of Employment
* SF50
* copy of ID card (for all locally hired DOD civilians)
* Area clearance message or dependent entry approval message listing school age dependents by name (Navy or Marines)
* Student's immunization record
 Sponsor or spouse must complete and submit the following documents:

* Sponsor/Pupil Registration (DS Form 600)
* Update for School Health Record (DoDDS Form 120)
* Present an updated copy of sponsor's orders if needed (Only required if the sponsor has received an extension of the original orders, or if the sponsor has been assigned to a different command within the commuting area)
