Adult - First Time Passport

NOTE:  For passport purposes, anyone age 16 or over is considered an adult. 

REQUIRED FORM:

REQUIREMENTS:

• Proof of U.S. Citizenship: Original or certified copy of Birth Certificate, original Consular Report of Birth Abroad, original Naturalization or Citizenship Certificate.  If you are over 16 years old and were issued a U.S. passport during your childhood, you may submit it as proof of U.S. citizenship.

• Photo Identification: Driver’s License or Photo ID card.

• One (1) recent photograph, in color, size 2” x 2”, with a white background and full front view.  Photo service is available at the Consulate.

• $135.00 U.S. Dollars.  Non-refundable

• Form of payment:  You can pay at the Consulate, in U.S. dollars or its equivalent in colones.   Also, International credit cards are accepted (Visa, Master Card, American Express).  Personal checks are not accepted.

SOCIAL SECURITY NUMBER:

You MUST provide your social security number on your passport application.  Applicants who fail to provide a valid social security number are subject to a $500 penalty enforced by the IRS. 

ADDITIONAL DOCUMENTS:

The Consular Officer may require documentation in addition to the items listed above.  In this case, the applicant has 90 days to provide the documentation.  After the 90-day period, the application will be considered abandoned and the applicant will have to begin the process again.

Adult – Passport Renewal

NOTE: Applicants whose previous passport was issued more than 15 years ago must apply for a first time passport.

• Previous U.S. passport (in good condition, not stained or ripped).

• One (1) recent photograph, in color, size 2” x 2”, with a white background and full front view.  Photo service is available at the Consulate.

• 110.00 U.S. Dollars non-refundable

• Form of payment:  You can pay at the Consulate, in U.S. dollars or its equivalent in colones.   Also, international credit cards are accepted (Visa, Master Card, American Express).  Personal checks are not accepted.

Adult - Lost or Stolen Passport

NOTE:  If you reported the theft of your belongings to the local police please bring a copy of the police report.

  • If you can wait 2 weeks for your passport please fill out form DS-64 and  refer to the instructions for first time adult passport
  • If you have an emergency travel to the United States you do not need to request an appointment, just come to the Consular Section, from 8:00 to 11:00 a.m.,  Monday through Friday (except U.S. and local holidays) and bring a copy of your travel itinerary.  You must justify your emergency with the Consular Officer.

Adult - Damaged or Mutilated Passports

DEFINITIONS:

Damaged Passport:  Ordinary wear or tear, carelessness, or accidental.

Mutilated Passport:  Intentional alteration of the document. 

 REQUIRED FORMS:

  • DS-82 - If all the biographical data and photo are NOT damaged. 
  • DS-11 - If the passport was mutilated or if the biographical data and/or photo ARE damaged. 

DOCUMENTATION NEEDED:

• Signed Statement: The statement must satisfactorily explain the circumstances regarding the damage of the passport book.

• Proof of U.S. Citizenship: Original or certified copy of Birth Certificate, original Consular Report of Birth Abroad, original Naturalization or Citizenship Certificate.

• Photo Identification: Driver’s License or Photo ID card.

• 135.00 U.S. Dollars non-refundable

• Both parents (or the child’s legal guardian(s)), as well as the child, must be present.

• Evidence of both parents’ identities: passports, driver’s licenses, or other photo identifications

• Child’s original Birth Certificate

• Proof of U.S. citizenship: Original birth certificate original or certified copy, original Consular Certificate of Report of Birth Abroad original, Citizenship or Naturalization Certificate original.

• Photo Identification: School diplomas, photo ID card, photographs of child clearly in the U.S.

• If the applicant is a minor born in the U.S., please bring family photos of the child in the U.S.

• Age progression photos should be submitted to establish the physical changes during the growth of minor children; these photos should range from the age since their last passport was issued to their current age.

• One (1) recent photograph, in color, size 2” x 2”, with a white background and full front view.  Photo service is available at the Consulate.

• $105.00 U.S. Dollars.  Non-refundable

• If neither parent is present in Costa Rica, a notarized written authorization in English from both parents to a third party is required.

Name Change, Data Correction and Limited Passport Book Replacement

Conditions:

• To change your name in your valid U.S. passport:
       
        IF your current passport was issued less than a year ago

• To correct a printing error in your valid U.S. passport

• To replace a limited validity passport, if your passport was issued less than a year ago.

REQUIRED FORM:

REQUIREMENTS:

• Valid U.S. passport

• Evidence to support the requested change, such as original marriage certificate, divorce or court order showing the change of name. 

• One (1) recent photograph, in color, size 2” x 2”, with a white background and full front view.  Photo service is available at the Consulate.
