Lost/Stolen Passports
 

Replacing Lost or Stolen Passports

All applicants whose passport has been lost or stolen must apply for a new passport in person. New passports require two weeks for processing. For more information, please visit the State Department’s website.

To replace a lost/stolen passport, the following are required:

1.  All available proof of identity such as: birth certificate, photo ID, photocopy of the missing passport, or expired U.S. passport

2.  Passport application form DS-11, available at the Consular Section or online.

3.  Police report and an affidavit (DS-64) regarding the circumstances of the loss/theft. See the information below for obtaining a Lao police report. Blank affidavit DS-64 forms are available at the Consular Section or online.

4.  One recent photo, 2x2 inches, taken on white/light color background. (The head size should be about one inch in length.)

5.  Fee of $135 in cash (minors under 16 years of age, $105)

Emergency passports may be issued for immediate travel with proof of an imminent departure. Emergency passports are valid for no more than 12 months (some may be issued with shorter periods of validity) and must be replaced by a full-validity passport printed in the U.S. It may take several hours to issue an emergency passport. Extra pages cannot be added to an emergency passport. You must return the limited validity emergency passport to obtain a full validity (5 or 10 years validity) passport.

Obtaining a Local Police Report

Lao authorities must verify and confirm that your passport has been lost or stolen and that you legally entered Laos. Upon realizing your passport has been lost or stolen, you must report the circumstances to local police and obtain a report of lost or stolen passport from local police. If you are unable to obtain this document at the location where the theft or loss occurred, it is possible to do so in Vientiane at the immigration police office. The Lao police report generally costs 10,000 to 20,000 kip and may take a day to process.

The Lao Immigration police office (021-212-520) is located on Saylom Road in Vientiane. Please see the map (PDF 155.39KB) for the exact location. Lao immigration is open Monday through Friday from 8:00-12:00 and 1:00-4:00.

Since you will depart Laos using a different passport from the passport with which you entered the country, you must rectify your immigration status with the consular section of the Lao Ministry of Foreign Affairs (MFA) before attempting to leave Laos. The consular section is located near the Batu Xay (021-254-175). Please see the map (PDF 155.39KB) for the exact location. The consular section is open Monday through Friday from 8:00-12:00 and 1:00-4:00. Please note that the office may close at 3:00 on some days.
