Madigan Healthcare System

Pharmacy Post Graduate (PGY1) Program

Site Description: Madigan Healthcare System is a Department of Defense tertiary care facility on Joint Base Lewis-McChord, located in Tacoma, Washington. The medical center was dedicated and opened to patients in 1992. It currently operates 220 beds (with a capacity to expand to 318), 14 operating rooms, and 15 radiology suites. One of the 100 Most “Wired” Hospitals in the U.S., the facility is supported by an automated transport system of robots, conveyor boxes and pneumatic tubes, as well as decentralized supply and medication point of use (POU) distribution system. The facility has over 1.2 million square feet of space in a nine floor inpatient nursing tower combined with a three floor ambulatory care clinic and four floor support complex. The facility averages almost one million outpatient visits a year with an average daily census of 150 inpatients. The pharmacy dispenses over 4,200 outpatient prescriptions, 1,500 unit doses and 500 sterile products a day.

Pharmacy Practice Residency: Madigan’s American Society of Health-System Pharmacists’ (ASHP) Post Graduate Year 1 (PGY1) Residency Program began in 1996, was fully accredited in 1997 and re-accredited in 2009. The program offers training positions for one active duty Army pharmacy officer and one civilian pharmacist each year. The Residency Program offers the unique feature of being able to follow patients across the continuum of care as they transition from inpatient wards to ambulatory clinics, all under one healthcare system; this continuity feature enhances the resident’s learning experience. Program objectives and goals are developed utilizing the ASHP Residency Learning System, providing structure and multiple opportunities for preceptor feedback during the program. Clinical experiences include acute care in Critical Care and Medical-Surgical wards, and ambulatory care in Internal Medicine, Family Medicine, Cardiology/Anticoagulation, Hematology-Oncology, Pharmacoeconomics, Warrior Transition/Polypharmacy, and Pain Management.

Practicum Environment: Decentralized clinical pharmacy services are provided in ambulatory, inpatient acute care and specialty settings. Clinical pharmacists provide pharmaceutical care to patients in Family Medicine, Cardiology, Intensive Care, Internal Medicine, Nuclear Medicine, Hematology-Oncology, Warrior Transition and Pain Medicine. These pharmacists manage patients with diabetes, hypertension, dyslipidemias, anticoagulation, chemotherapy/oncology, polypharmacy, chronic pain, intensive care, acute care and infectious disease.

Pharmacy residents have opportunities to work with pharmacy students. Additionally, the centralized inpatient pharmacy provides learning experiences in sterile products, drug delivery technology and nutrition.
