In order for us to serve you, please print out and fill out my Privacy Authorization Form and then fax or mail it to my office.
On the waiver, please include all relevant information to your problem, including your name,
address, phone number, Social Security number, Medicare number, Alien number, etc. Please
also include documentation of your correspondence with the agency. Also helpful is the inclusion
of a letter describing the situation and detailing your request.
