To apply for a new Social Security Number or make a correction to your data:

1. Inform yourself of this application by reading the information found at http://www.socialsecurity.gov/online/ss-5fs.html

2. Download and complete form SS-5 FS (163KB, pdf)

3. Should you need to have a personal interview or have documents certified, please arrange an appointment with the American Citizens Services Section at the Embassy in Vienna via link detailed above.

4. All other completed applications with appropriate documentation, are to be forwarded to the Federal Benefits Unit in Frankfurt:
