Overseas Deployment Screening

To clear for Overseas Travel you will need the following:

  1. Children:

    Current physical (within the last year).

  2. Women:

    Current well women's exam (within the last year).

  3. Men:

    Current physical (within the last year).

  4. DA Form 5888 with top portion completed and signed off by sponsor's Personnel Section.
  5. Dependents outpatient records.
  6. Children under the age of 6 years that aren't enrolled in school.

Note: Clearace — the DA Form 5888 can only be signed off by an EFMP Physician.

Criteria for enrollment

  • Potentially life threatening conditions including but not limited to asthma (recurrent wheezing treated with any medication) within the past 5 years, sickle cell disease, and insulin dependent diabetes.
  • Chronic (greater than 6 months of continuous care or multiple episodes of care) outpatient mental health treatment over the past 5 years or impatient mental health services within the past 5 years.
  • Any level of mental health services required at the present time or projected for the future.
  • Attention Deficit Hyperactivity Disorder requiring management and treatment by a physician, mental health care provider or counselor.
  • Soldiers who have family members that require intensive follow up support (such as high risk newborns and patients with diagnosis of cancer within the past 5 years).
  • Soldiers who have family members that require education services (including medically related services) that are specified on an Individualized Education Program from 3-21 years of age or early intervention services specified on an Individualized Family Services Plan from birth to 3 years of age.

Who Can Attend

  1. No children
  2. Anyone who wants to know about the EFMP
  3. Sponsor/spouse of enrollee

Updating Enrollment

Attending the Enrollment Briefing is not required; go to one of the EFMP offices and pick up the necessary summaries/forms.

Coding

William Beaumont Army Medical Center is a coding center. No backlog in coding. The soldier will receive his completed coding within one week after the medical/educational summaries are turned in.

Member of the Special Needs Resource Team(SNRT):

The team meets two times a month, or when an emergency occurs. The team discusses the needs and training of staff for special needs children when they are left in the care of any post day care center or day care homes.

Counseling

Assisting soldier/dependents with difficult assignments, getting necessary services, scheduling appointments for specialty clinics.

Oversight Committee

The Oversight Committee, on post and off post organizations, meet quarterly to discuss changes taking place at Fort Bliss and in the El Paso community. A representative from all the school districts, Early Intervention Services, JAG Office, Civilian Personnel, Housing, Engineers, MILPO, State Department, EFMP/WBAMC and Community Service Center attend this meeting.
