Application for Additional Visa Pages

Note: Read this paragraph first

If you submit your application in Papua New Guinea, please read carefully the information outlined below before you print and complete the forms. The information printed on the forms does not apply to the applicants in Papua New Guinea. You must submit all pages of the applications, except as otherwise stated. Your application will not be accepted unless the required pages are submitted.

You may request additional visa pages to be added to your valid U.S. passport by:

  • Completing, signing and dating an application (Form DS-4085) in black ink;
  • Submitting your application and your valid passport via mail, or in person at the U.S. Embassy, Port Moresby.

Signing the application:

  • Applicant age 14 or above: sign your own application
  • Applicant age 13 or below: both parents, or legal guardian(s), must sign for the child (refer to the information below

Return or collection of your passport:

If you do not wish to collect your passport in person, please provide a pre-paid returned envelope from DHL or TNT courier services with your application. For applications submitted in person by an agent other than the applicant, a written and signed authorization of the applicant is required. The agent must present his/her valid photo-identification (e.g. driver's license, passport).

Note:

Additional visa pages cannot be added to an expired, damaged or mutilated passport.

Application of a child under age 14

  1. If the child is age 13 or below, both parents, or legal guardian(s), are required to sign the child's application form. The signing parents', or legal guardian's, valid identifying documents (e.g. passport, driver's license) and evidence of parentage, or legal guardianship (e.g. the child's birth certificate, court orders), must be submitted with the application.
  2. If one of the parents is not available to sign the child's application (for instance, the parent is traveling abroad, or legally separated/divorced), the absent parent must complete and sign a Statement of Consent (Form DS-3053). This form and a clear photocopy of the signing parent's valid identification (e.g. passport, driver's license) must be notarized by a Notary Public or a Commissioner of Oath. One parent's signature is acceptable ONLY if the signing parent or legal guardian has exclusive custody/guardianship of the child. Documented evidence must be submitted (i.e. court orders, death certificate of a deceased parent, etc) with the application.

Note:
All documents submitted in support of your application (as outlined in paragraph number one above) must be either originals or copies certified by the state or federal authorities, bearing the official's seal and signature, and must not be laminated. Hospital records/certificates will not be accepted. Photocopies of documents notarized by a Clerk of Court, Commissioner of Oath, solicitor or Notary Public are not acceptable. An official written translation is required for any documents issued in languages other than English. The submitted documents will be returned to the applicant.

Conditions for downloading passport application forms

  • Submit ALL pages of the application forms. Your application will not be accepted unless all pages are submitted.
  • The application forms must be printed clear and sharp; forms that are blurred or illegible will not be accepted.
  • Print the application forms on A4 size plain white photocopy paper with a matte surface. Thermal paper, dye-sublimation paper, special inkjet paper, and other shiny papers are not acceptable.
  • Print the application forms only on one side of a sheet of paper, not front and back.
  • The forms must be printed in black on white paper, NOT in colour print or on coloured papers.
  • Forms printed with dotted or broken lines, or blackout portions will not be accepted.

Do not shrink the size of the forms for printing. Please make sure that the photograph area on the application form is 5 cm X 5 cm in order to attach the photograph correctly.

  • Each family member seeking a passport or citizenship service may request an appointment-but it is not mandatory.  Please schedule your appointment on-line after assembling the documents and forms listed above.

    If you choose to schedule an appointment, you will receive a Confirmation Sheet.  Please print the Confirmation Sheet and bring it with you to the interview.  Your application form must be completed prior to your arrival on your appointment date.
