Damaged or Mutilated Passports

If your passport has been damaged or mutilated, you must apply in person for a new passport.

Adults (age 16 and older)

  • All applicants must apply in person.
  • Present damaged/mutilated passport.
  • Present proof of U.S. Citizenship (If identifying information on the passport is illegible you must submit proof of citizenship other than the damaged/mutilated passport.)
  • Present one valid proof of ID
  • Provide one passport photo
  • Complete form DS-11
  • Bring one self-addressed LetterPack 500 envelope (for persons using APO address, one self-addressed envelope)

Children under 16

Passports for Children:

As of February 1, 2008, the two-parent consent requirement for all minors' passport services was extended to include all children under age 16. Both parents are required to be present to provide signatures at the time of application. For more information please see here.
