Basic Procedures:

We service our customers on a walk-in basis. Proper identification and the appropriate documentation (see below) are required before services can be rendered. Sponsors must accompany their family members at all times. However, the facility does accept power of attorneys in the absence of the sponsor.

Hours of Operation:

Monday thru Friday 0730 to 1630.

ACTIVE DUTY ID CARD:

  1. Expired: need previous issued active duty ID card and Extension or Reenlistment documents.
  2. Grade Change: Promotion Orders.
  3. Lost Active Duty ID Card: SGT AND BELOW REQUIRE COUNSELING STATEMENT SIGNED BY COMPANY COMMANDER OR 1SG, must have some other form of picture ID; if the soldier has no picture ID, must have social security card and birth certificate.

CIVILIAN AND CONTRACTOR ID CARDS:

To process a civilian and contractor Identification card, the employee would need to have two forms of identification (state Id, old civilian ID card DD Form 1602, etc). They will also need to have an AKO account (must know email address to AKO account). Civil service employees that are not in the DEERS data base must bring a DD Form 1172-2. All Contractors will need a DD 1172-2 upon issuing of the Identification card.

DEERS ENROLLMENT:

Initial DEERS ENROLLMENT requires mandatory disclosure/verification Social Security Numbers of all beneficiaries of the Uniformed Services´ health care system. Failure to disclose a SSN when enrolling in DEERS will result in loss of eligibility for medical care benefits in Military Treatment Facilities. Additionally, you must have certified copies of Marriage Certificate for spouse; Divorce Decree if beneficiary was previously married; certified Birth Certificate for children/step-children; hospital confirmation of birth letter can be used in place of a birth certificate for a child under the age of one year; final adoption decree if the family member was adopted. Everyone must be enrolled in the DEERS Data Base before an ID Card can be issued or before medical treatment can be given.

DEPENDENT ID CARD:

  • Initial Issue: Certified copy of marriage certificate, birth certificate, Social Security Card, and DD Form 214 if prior military.
  • Expired card: Previously issued ID Card and Sponsor´s presence to sign application. If sponsor has been previously married, a divorce decree must be sighted before a new spouse’s ID Card can be issued. Dependent can have General Power of Attorney if sponsor is not available or DD Form 1172 not over 90 days old from date sponsor signed the application. Dependent ID Cards can be renewed 90 days from expiration date on ID Card, with the exception of a dependent that is turning 65 years of age. This ID Card can be renewed on the first of the month of the 65th birthday or any day thereafter.
  • DEPENDENTS 21 YEARS OF AGE remain eligible for an ID Card if enrolled in an accredited college as a full time student and pursuing a degree (associate, master, bachelors, etc...). Dependent must have a letter from the Registrar´s office on one of the school´s letterhead. The letter must state that the dependent is a full time student, how many hours attending, and an anticipated graduation date. The student’s ID Card will be issued until the 23rd birthday or graduation date, whichever is first.
  • ILLEGITIMATE CHILD: Documentation needed is a Social Security Card, a court ordered paternity statement or a court order ordering child support.

DEPENDENCY DETERMINATION: Dependency determination can be done for a child over the age of 21 years of age who is incapacitated; parents, and parent-in-laws, providing that the sponsor is supporting these dependents with at least 50% of their living expenses.

  • Incapacitated Child 21 years of age: DD FORM 5639-R which can be obtained from the Post ID Card Section (Building 465), Birth Certificate, Medical statement verifying that dependent’s medical incapacitation is permanent or temporary, Retirement orders (if sponsor is retired), letter from Social Security office verifying if child is receiving social security benefits, and, if so, whose social security number the benefits are being received under.
  • Parent/Parent-in-Law documentation needed: DD Form 137-3 which can be obtained from the Main Post ID Card Section, sponsor’s birth certificate if it is the sponsor’s parent, spouse’s birth certificate and marriage certificate if it is the spouse’s parent. If the parent has medical problems, a medical statement is required. If the sponsor is retired, retirement orders or retirement DD 214 is required in addition to the previously listed documents.

DISABLED VETERAN:

A disabled veteran will need the following documentation before an ID Card can be issued: A letter from the VA office stating that he/she is 100% disabled service-connected with a statement whether the veteran is scheduled for future exams, DD 214, and picture ID.

IDENTIFICATION TAGS:

The soldier will need a DA Form 4187 signed by his/her Commander or First Sergeant. If a Commander has multiple ID Tag requests, send request to Chief of Personnel Processing Center. Must have the following info for ID Tags: First, middle initial, and last name, Social Security Number, Blood type, and Religion.

RESERVE/GUARDRESERVE:

Initial issue for Reserve/Guard Reserve requires a DD Form 1172 verified by the reservist´s unit and a picture ID. A DD Form 214 can be used if the DD Form 214 has the reserve obligation date on it.

RETIRED ID CARDS:

Initial Issue: Retirement orders or retirement DD 214; if on transition leave, DA Form 31 verifying leave dates, and sponsor’s Active Duty ID Card is required. The initial Retirement ID Card may be requested one calendar day prior to retirement date. Replacement ID card: Picture ID only if sponsor is enrolled in the DEERS Database. If not enrolled in the DEERS Data Base, a picture ID, retirement orders, or retirement DD Form 214 is required.

UNREMARRIED FORMER SPOUSE:

Unremarried former spouses can continue to keep their Dependent ID Card privileges providing they meet the following criteria: The member served at least 20 years creditable service for retired pay, the marriage lasted at least 20 years, and the marriage and service overlapped by at least 15 years. The number of years overlap in marriage and service determines the extent of benefits authorized.
Documents needed: Certified Marriage Certificate, Divorce Decree, sponsor’s retirement DD Form 214 or Statement of Service and former spouse’s picture ID.
