Name Changes
If you have changed your name, you may apply to have this change reflected in your passport. Please note that as of September 26, 2005, U.S. passports will no longer be amended to change items appearing on the data page; you will therefore apply for a new passport in your new name.
If the change is within one year after issuance, to apply please provide the following:
- Current passport;
- completed form DS-5504;
- two passport photographs (taken within the last six months);
- documentation of name change (e.g. original or copy of marriage certificate, divorce decree or court order).
Fees: no fee is charged for this service.
If the change is more than one year after issuance, please follow regular procedure for passport renewal and include documentation of name change (e.g. original or copy of marriage certificate, divorce decree or court order.)
