Request for a Social Security Card
If you are applying for a Social Security card for the first time for someone age 12 or over, please contact us at Panama-FBU@state.gov for an appointment. An appointment is not required for replacement cards or for new applications for children under age 12
- Original literal copy (copia integra) of the birth certificate, if the applicant was born in Panama. Original birth certificate if born in another country
- Current U.S. Passport
- Document reflecting your SSN (Only for replacement.)
- Fill out form SS-5.
- If the card is for a minor child, one of the parents should bring his/her current passport or alien card & passport.
- If the card is for a minor and the minor can sign his own name, he should sign his application.
- If the card is for a child age 12 or older, it is required that he/she be present for an in-person interview.
- If the applicant is 12 years or older and never applied for a social security number, he/she will need to bring the school transcripts for all schools attended outside of the United States.
- If you are requesting a name change, you should present documentation with the former name used and the new name.
