Lost or Stolen Passports
 

Lost or stolen passports must be reported to the Consular section at the time of application for replacement. You do not need an appointment to apply for a replacement of a lost or stolen passport. You may fill out an application, Form DS-11, and a Form DS-64, Statement Regarding Lost or Stolen Passport on line and print it out to bring with you to the Embassy or you may take a form from the waiting room in the American Citizen Services Unit and fill it out before coming to the window. Make sure you fill out the right form. The loss or theft of your passport must be reported to the nearest police station in the area where the loss or theft occurred, along with a description of any other items missing in the same incident.

You will need to bring a:

  • Passport-sized photo (5cm x 5cm)
  • Copy of the police report
  • Fee of $135 (age 16 and older) or $105 (15 and younger) to replace your lost or stolen passport.

Replacing Peruvian Immigration Entry Stamps

Peru's Dirección General de Migraciones y Naturalización (DIGEMIN) has changed the procedure for travelers exiting Peru with an emergency passport.  In cases of passports that have been lost or stolen, the passenger will not have the entry stamp with which they entered Peru.  Therefore they must "transfer" the entry stamp to their new passport before they are allowed to pass through immigration.  Previously, travelers were able to complete this process at the Jorge Chavez Airport in Lima. Now, travelers must take their new emergency passport to DIGEMIN headquarters at Av. España 730, Breña, Lima, open weekdays from 8:00 a.m. to 12:30 p.m. in order to pay for and process the transfer. 

We recommend that travelers with a lost or stolen passport arrive at the Embassy at 8:00 a.m. in order to receive their emergency passport as soon as possible.  After travelers receive their emergency passport, they will have to go to DIGEMIN headquarters to complete the stamp transfer process.  Because of the restricted time frame at DIGEMIN headquarters, travelers may not be able to travel on the same day as the issuance of their emergency passport.  If you have questions or concerns, please contact us at LimaACS@state.gov.

Travelers should be aware that Peruvian immigration authorities do not permit departure from Peru until all immigration matters, including fines and overstays, have been put in order.
