Consular Outreach Visits for U.S. Citizen Services

In order to better serve U.S. citizens living throughout the Eastern Caribbean, the U.S. Embassy in Barbados makes every effort to send a Consular Officer to countries in our Consular District approximately twice a year to offer certain American Citizen Services such as accepting applications for U.S. passports and Consular Reports of Birth Abroad, and provide notarial services.

You must schedule your appointment online, print the confirmation page and bring it to your appointment with the Consular Officer at the designated location/time.   After scheduling your appointment, please review the instructions below to ensure that you are well prepared.  Remember it is critical that you bring both originals and photocopies of all requested documents.

Once you have made your appointment, please follow the instructions below:

U.S. Passport Application Procedures

Items Required for all Passport Applications

• Your most recent U.S. passport; or
• Proof of U.S. citizenship (for example, an original U.S. birth certificate, naturalization certificate, or consular report of birth) and a valid government issued photo ID; and
• Two (2) recent photographs (5cms x 5cms or 2"x2") of the full face with a light background;
• Fees payable in cashier's check or international money order (no cash) - made payable to the U.S. Embassy; and
• Your Social Security card and a copy of it (if you do not have a Social Security number or do not have a card, please inform us during your appointment); and
• Photocopies of all of your documents must be presented; (bring original + photocopy).  This includes photocopies of parents' passports (or other identification) when applying for a minor.
• For maximum security and speed of service, you must submit a prepaid courier service Air Waybill from (LIAT, DHL or FedEx) to be used for the return of the passport and any original supporting documents.

Application forms are available on the State Department's website. Please bring the completed application to your appointment. 

Minors Under Age 16

• Each minor child must appear in person;
• Both parents must appear together and sign Form DS-11 in front of the Consular Officer or
• One parent appears and submits the other parent's notarized Statement of Consent (notarized Form DS-3053, not more than 90 days old, must be accompanied by a photocopy of absent parent's passport, driver's license or I.D. card) or
• One parent appears, and submits evidence of SOLE authority to apply (such as one of the following):
  
  • Child's certified U.S. or foreign birth certificate lists only applying parent; or
  • Consular Report of Birth Abroad lists only applying parent; or
  • Court order granting sole custody to the applying parent (unless the child's travel is restricted by that order), or
  • Adoption decree (if applying parent is sole adopting parent), or
  • Court order specifically permitting applying parent's or guardian's travel with the child, or
  • Death certificate of non-applying parent.
• Completed Form DS-11;
• The child's Social Security card and a copy of it (if the child does not have a Social Security number or does not have a card, please inform us during your appointment); and
• Fee of U.S. $105.00
• For maximum security and speed of service, you must submit a prepaid courier service Air Waybill from (LIAT, DHL or FedEx) to be used for the return of the passport and any original supporting documents.

  Applicants Age 16 to 17

• Each minor child must appear in person and execute his/her own passport application;
• The parent or guardian accompanying the child must also submit his/her own passport (and photocopies of the ID page) for identification purposes;
• Completed Form DS-11;
• The child's Social Security card and a copy of it (if the child does not have a Social Security number or does not have a card, please inform us during your appointment); and
• Fee of U.S. $135.00
• For maximum security and speed of service, you must submit a prepaid courier service Air Waybill from (LIAT, DHL or FedEx) to be used for the return of the passport and any original supporting documents.

Applicants Age 16 and Over

• Each applicant must apply in person if the previous passport was issued when they were under the age of 16;
• Completed Form DS-11;
• Your Social Security card and a copy of it (if you do not have a Social Security number or do not have a card, please inform us during your appointment); and
• Fee of U.S. $135.00
• For maximum security and speed of service, you must submit a prepaid courier service Air Waybill from (LIAT, DHL or FedEx) to be used for the return of the passport and any original supporting documents.

Applicants over the age of 16 have the option of applying by mail (or appearing in person using form DS-82) provided they fulfill all of the following criteria: 

• The last passport was issued when the applicant was 16 years of age or over; and
• The most recent passport was issued within the last 15 years; and
• The most recent passport was not limited (i.e. it was issued for its full five or ten-year validity); and
• The applicant is able to submit their last passport; and
• The applicant is not requesting a change in the passport data, other than a name change due to marriage.
• Fee (for form DS-82) of U.S. $110.00

Lost or Stolen Passport

If you lose your passport, a replacement can be issued upon presentation of proof of identity and U.S. citizenship (i.e. original birth certificate, previous passport or photocopy of missing or previous passport, original naturalization certificate). If you do not have proof of citizenship, you must bring with you an American family member or American friend with his/her passport willing to execute an affidavit attesting to your nationality.  If your passport has been stolen, first report the theft to the local police and obtain a police report.  The report MUST be given to the Consular Officer at the time of the application.

U.S. Consular Report of Birth Abroad  -- Registering Overseas Births of U.S. Citizens

Application Procedure

When a child is born overseas to an U.S. citizen parent, or parents, certain documentation is required in order for the child to be registered as a U.S. citizen.  Once registered, the child can receive his/her "Report of U.S. Birth Abroad" and a U.S. passport can be issued.  The children can also apply for a Social Security number. 

For registration, the following items are necessary:

• Original birth certificate issued by the local authorities at the place of birth.
• Original marriage certificate of the child's parents.
• Original documentation of the termination of any previous marriages of the child's parents (death or divorce certificates).
• Parent(s)' U.S. passports or other evidence of U.S. citizenship
• Non U.S. citizen parents must also produce their passports
• Application for Consular Report of Birth Abroad (PDF 52K), (Form DS-2029) available online. Do not sign the form until requested by a consular officer.
• Two identical and recent photos taken full face with a light background (5cm x 5cm or 2"x2").
• Affidavit of Physical Presence (PDF 165 K) - Form DS-5507. See below for more information
• USD$100.00 for registration of the birth abroad and
• USD$105.00 for the U.S. passport.
• Social Security Application Form - SS-5, available online.  Please print and complete by hand.

Affidavit of Physical Presence (Form DS-5507) - The U.S. citizen parent should complete, but not sign the DS-5507 form.  Parents who were not married at the time of the child's birth also need to bring evidence of their physical presence together at the time of the child's conception (i.e. passport stamps, military travel orders) as well as proof of their relationship prior to the child's birth (i.e. letters, photos).  If only one of the parents is a U.S. citizen, he/she must bring proof of physical presence in the United States prior to the child's birth.  Primary evidence that may establish presence in the U.S. includes:  Transcripts from high school and/or college, and Wage Statements (W-2), DD214 Separation Statement (for Military Members). Secondary evidence includes:  Tax returns, airline ticket stubs, former/current passport showing entry/exit stamps.

Please be reminded that photocopies of ALL of your documents MUST BE presented.

 Social Security Cards for first time applicants: 

• Form SS-5 - please print and complete by hand
• Applicant's U.S. passport
• Applicant's Birth Certificate
• A personal Interview is required for an applicant who has never had a Social Security Card
• Parents' Passports (only required for children under age 18)
• First time applicants who are 12 years and over - Proof of residence outside of the United States from age 12 until present (e.g. school records, job letters, etc.)

Please be reminded that photocopies of ALL of your original documents must be presented.
