Lost or Stolen Passport
The Embassy encourages you to report the loss or theft of your passport to local authorities, and to obtain a police report.
Obtaining a replacement passport is a two-step process.
1. Report the loss or theft of your valid passport immediately to the Embassy using form DS-64.
2. Apply for a replacement passport as a first time passport applicant.
If you need to travel for an emergency, there is no need to make an appointment. Simply come to the Embassy with form DS-64 and the requirements of applying for a passport for the first time. Applicants requesting an emergency passport will be required to provide proof of immediate travel plans. If your travel is not an emergency, please make an appointment by clicking here.
