Visa Application Form DS-160

All visa applicants must use the electronic DS-160 visa application form and must present the DS-160 one-page confirmation sheet with the bar code at the time of the interview appointment.

Please note: Accurate data entry is imperative. Applicants who enter incorrect biographic information including family name (surname/last name), first name (given name), gender, date of birth, or place of birth, or who do not enter all aliases (other names previously used, i.e. maiden name, previous married name) will be required to correct their visa application forms online and then return to the Consulate.

Applicants will also be required to correct their DS-160 application form online and then return to the Consulate if the DS-160 application form contains the wrong visa classification at the time of the interview.

Please ensure you select the correct visa category for your proposed travel under the section “Travel Information” of the DS-160 when asked for the purpose of trip. The DS-160 application questions change based on your purpose of travel. Most visa categories, such as temporary workers (including performing artists like musicians and singers), journalists, crew members or investors are actually a subcategory of the choice “other” in the drop-down menu. Official business trips on behalf of foreign governments or international organizations are A and G visas, also subcategories of the choice “other”.
