About the Program:

Care Management Provided:
- Clinical support as newly returning combat Veterans travel through assessment and mutually agreed upon care plans with the overall goal of assisting the Veteran achieve and maintain his or highest level of functioning possible.
- Training and consultation to medical center staff with regard to the unique needs of OEF/OIF/OND combat Veterans, as well as resources available to this population.
- Conduct and support outreach activities, often collaborating with other medical center programs, as well as community agencies, in support of returning combat Veterans.
