Passport Application Requirements

The U.S. Consulate can accept passport applications for passport renewals, first-time applicants, and replacements for lost or stolen passports. We can also add up to two additional sets of visa pages and process corrections to passports.

Please review the requirements below carefully prior to scheduling your appointment.  On the day of your appointment, please bring completed forms to the Consulate.  Failure to complete all required documents may result in having to return to the Consulate at a later date.

U.S.  Passport Application

 This form must be filled out completely in blue or black ink using block letters.

  • Renewals / Lost or Stolen Passports / First-time Applicants: applicants should fill out the electronic passport application found at https://pptform.state.gov/ Before filling out the form, make sure you have a printer available as you will be required to print the form upon completion.
  • To Add Visa Pages: complete Form DS-4085 (PDF, 59.2KB). Please note that we cannot add extra visa pages to a valid passport more than two times.
  • To Change the Name or To Correct Passport Information: if you are applying within the first year of issuance, please fill out form DS-5504.  Changes requested after the first of issuance must use electronic passport application found at https://pptform.state.gov/

Photographs

One color or black & white photo measuring 2" x 2" (see size on the application) printed on unglazed paper and taken within the last six months reflecting your current appearance.  The photograph must show a full front view of the applicant's face with a white background.  Hats and dark glasses should be removed for the photos.  We cannot accept instant photos.  Infant pictures should be taken with their eyes open.  For additional information on passport photographs, click here.

Passport Fees

 (New Schedule of Consular Fees took effect on July 13, 2010.  Please see below or click here for a complete list)

  • US$ 135 for first-time adult applicants and to replace lost/stolen passports.
  • US$ 105 for first-time applicants under the age of 16.
  • US$ 110 for adults renewing full validity passports within 10 years of issuance.
  • US$ 82 for adding additional visa pages.
  • NO FEE for passport amendments (corrections) if they are supported by the form DS-5504 and it has been less than one year from the issue date of the previous passport.  These amendments include:
  • Name change, by marriage or any other legal means;
  • Incorrect information was printed in your passport, when this information is different from what you provided on your passport application form and supporting documents; or,
  • If you are replacing an emergency or limited validity passport within one year of the date of issuance.
  • Any passport amendment not included in the previous topic will be charged depending on the type of service required.

Payment of fees may be made by cash, credit card (Visa, MasterCard, American Express, Discover and Diners Club) or certified checks from an American bank.  No personal checks are accepted.

Proof of Citizenship

For example, a U.S. birth certificate, consular report of birth abroad, naturalization certificate, or prior U.S. passport.

Proof of Identity with Photograph

For example, a driver's license, prior passport or other government issued ID.  Children and first-time applicants should bring a series of pictures from birth to present.  In the case of infants and children, progression photos should establish a clear likeness between the applicant’s current appearance and the photograph used to obtain the previous passport or proof of citizenship.  The Consulate General recommends at least two photographs per year.  Acceptable photographs should show a full front view of the applicant’s face with eyes open.

Additional Requirements for Children Under Age 16

a. Original Birth Certificate.  For children under 16 years of age, an original copy of the U.S. birth certificate must be provided.  For children born in Ecuador, please provide an original copy of the Ecuadorian birth certificate certified by the Ecuadorian Civil Registry, and the Consular Report of Birth Abroad.

b. Progression Photos.  In the case of infants and children, progression photos should establish a clear likeness between the applicant’s current appearance and the photograph used to obtain the previous passport or proof of citizenship.  The Consulate General recommends at least two photographs per year.  Acceptable photographs should show a full front view of the applicant’s face with eyes open.

c. Parental Consent for Children Under Age 16.  Since July 2, 2001, the Code of Federal Regulations, Title 22, Section 51.28 provides that U.S. passport applications for children under the age of 16 require the consent of both parents (or legal guardians).  Thus both parents must be present at the time of the passport application.  Parents must provide valid photo IDs and the child's original U.S. birth certificate or consular report of birth abroad.  If only one parent is able to come, the applying parent must provide one of the following:

i. A notarized, written statement of consent authorizing passport issuance for the child from the non-applying parent (Complete Form DS-3053) and a clear, front and back, photocopy of the non-applying parent's valid identification (passport, driver's license, or other government-issued identification card); or,

ii. Certified copy of foreign birth certificate listing only the applying parent; or,

iii. Report of Birth Abroad (Form FS-240) or Certification of Birth Abroad (Form DS-1350) listing only applying parent; or,

iv. Court order granting sole legal custody to the applying parent (unless child's travel is restricted by that order); or,

v. Adoption Decree (if applying parent is sole adopting parent); or,

vi. Court order specifically permitting applying parent's or guardian's travel with the child; or,

vii. Judicial declaration of incompetence of non-applying parent; or,

viii. Death certificate of non-applying parent.

Note: The consular officer may request additional documentation, including additional progression photos or other items which may not be listed.  If a consular officer is not satisfied with a statement of consent and is not satisfied with additional documentation and explanations, then the consular officer may refuse to issue the passport.

Make an Appointment

  • American Citizen Services accepts applicants for passports by appointment on the second floor of the Consular Section at the U.S. Consulate General in Guayaquil.

    For faster service, please arrive promptly at your scheduled appointment time with your application forms completed, required documentation in order, one passport-sized photo, and an acceptable form of payment for the passport service.

    Appointments are offered Monday through Thursday, 1:00 pm to 3:00 pm. Appointments must be made using our online appointment system, available here.

Important Notes

  • Dual Nationals
    Dual nationals are reminded to plan travel to Ecuador using U.S. passports valid for at least six months from their intended date of return to the United States.  Dual nationals may enter the host country on a non-U.S. passport, but Ecuadorian immigration will require that dual-national travelers present a U.S. passport with a minimum of six months validity remaining when passing through exit controls to depart Ecuador and return to the United States.

    Passport Processing Time
    Due to the processing time required to produce a new passport and ship it from the United States, applicants should expect that it will take approximately 10 business days to receive a new U.S. passport after the application has been approved.  Passports can be picked up from our offices any day from Monday through Thursday from 8:00 am to 11:00 am. except for holidays.

    Replacing a Lost/Stolen Passport
    Applicants seeking to replace a lost/stolen passport should complete the “Statement Regarding Lost/Stolen Passport” (Form DS-64 PDF - 44kb).   At the time of application for a new passport, they must have reported the loss to local police and provide a copy of the police report.  They must also obtain a report from Ecuadorian Immigration (known locally as a “movimiento migratorio”) showing when they entered Ecuador using the lost/stolen passport.

    Emergency Travel
    In specific cases, such as a medical emergency, limited validity passports may be produced locally at the discretion of the consular officer.  Please note that lack of planning on the part of travelers who have already purchased airline tickets or tour packages does not constitute an emergency.  Contact the Consulate for more information.
