Individuals applying for a non-immigrant visa at the American Consulate General in Guayaquil are required to submit the DS-160 application form.

Applicants for K Visas should continue to use the DS-156 and DS-157 (PDF - 334kb) forms.

Notice: Visa  applicants must  print out the confirmation page containing a bar code and bring this to the interview.  It is not
necessary to print out the application form.

If you have left any questions or space blank you will not be admitted for your interview.

We only accept laser printed confirmation pages.  Dot matrix printed pages containing a bar code, will be automatically rejected because they cannot be read by our scanners.
