PGY1 Pharmacy Residency Program - Ambulatory Care Track

This ASHP-accredited PGY1 Pharmacy Residency – Ambulatory Care Track provides clinical and teaching experience in patient care management in an environment actively developing pharmaceutical care focused in the ambulatory care setting.

The goal of this one year post-graduate program at the San Francisco VA Medical Center is to train practitioners to be competent direct care providers in a variety of clinic settings including:

  • Anticoagulation
  • Medical Practice
  • Hepatitis C
  • Home Based Primary Care
  • Mental Health
  • Infectious Disease
  • Congestive Heart Failure
  • Diabetes Education
  • Oncology
  • Medication Teaching Clinic

Additionally, the resident will gain experience in acute patient care, drug information and drug use policy development, practice management, teaching, and research project development and management.

Acute Patient Care

During this six week experience, the Ambulatory Care Pharmacy Resident is part of an internal medicine team and is responsible for monitoring drug therapy, determining optional therapeutic outcomes, performing medication reconciliation, providing drug information, and counseling patients about their medications.

Practice Management

The Ambulatory Care Pharmacy Resident will gain experience in the management of various areas of the Pharmacy Service.  The resident will learn from pharmacy managers, specialists, and generalists about the development of their particular practice setting.  The resident will be given opportunities to improve communications skills and proficiency in time management. The resident will be actively involved in a patient safety or performance improvement project.

Drug Information and Drug Use Policy Development

The resident receives training in drug information and may complete an elective in drug information or poison control. The Pharmacy Service is an active participant in all medication management policy decisions.  The resident will have the opportunity to perform drug evaluations for local, regional, and national formulary committees or revise drug use criteria for local and regional formulary committees, and perform drug usage evaluations.  Drug information resources include on-line computer databases. The resident is responsible for evaluating and reporting adverse drug events and medication errors.

Teaching

The Ambulatory Care Pharmacy Resident precepts advanced pharmacy practice experience students from UCSF and the University of the Pacific.  Other teaching opportunities include nursing in-services, house staff presentations, pharmacy conferences, and patient education classes.

Other Experiences

The resident participates in pharmacokinetic seminars and receives formal training in cardiopulmonary resuscitation.  Each resident has the opportunity to participate as a member of the code blue response team. The residents complete a residency project which is presented at the Western States Conference for Pharmacy Residents, Fellows and Preceptors. Additionally, the resident is offered the opportunity to take a variety of off-site electives.

  • Application for Health Professions Trainees – VA Form 10-2850d 
  • Declaration for Federal Employment – OF 306 
  • Current curriculum vitae (please include details of rotation activities, future rotations, any teaching or community service activities)
  • Official College of Pharmacy transcript
  • Your e-mail address, mailing address, and phone number where you can be reached after January 5.
  • Statement of purpose (not to exceed one page)
  • Three letters of recommendation. At least one of your letters of recommendation must be from a preceptor with whom you have had a clinical rotation. We do not accept the standardized reference available in PhORCAS. Please give individuals writing letters of recommendation a copy of the "PGY1 Ambulatory Care Track - Reference Writer Instructions" letter.

An on-site interview is required and is usually scheduled for late January or February.

Qualifications

Candidates must be United States Citizens, possess a Doctor of Pharmacy degree from an accredited school of pharmacy, have significant clinical clerkship experience, and be licensed by January 1 of the residency year.

Educational Leave and Benefits

Each resident receives paid educational leave to attend the ASHP Midyear Clinical Meeting, the CSHP Seminar and the Western States Conference for Pharmacy Residents, Fellows and Preceptors.  Benefits include 13 days vacation, 13 days of sick leave, 10 holidays and optional health care insurance.
