Visa pages can be added to a U.S. passport by submitting a completed Form DS-4085: Application for Additional Visa Pages.  The fee for this service is US$82, payable in U.S. dollars or Brunei dollars equivalent in cash only.

You mustmake an appointment viaU.S. Citizen Services Appointment System to submit Form DS-4085 at the Embassy. If you appoint a representative to bring your passport to the Embassy, please ensure the representative brings his/her valid photo I.D., a signed authorization letter from you, and your signed Form DS-4085.

When applying for visa pages for a child under 16, at least one parent must sign the application form.  It is not required that the child appear in person.  However, the consular officer reserves the right to request further documentation if there are concerns.

NOTE: The Bureau of Consular Affairs recommends adding no more than two sets of visa page inserts to a passport.  Beyond two additional sets the added thickness may result in damage to the binding of the book during its validity.
