Notes for Auditors of LSC Recipients: • The system is not currently accepting Summary Report Forms for FY 2012 audit reports. Please check back March 15, 2013 to submit Summary Report Forms.
• Finding information to be reported includes all findings of noncompliance relating to all regulations covered by the Compliance Supplement and all reportable conditions (whether material or not material).

The Summary Report Form (SRF) is designed to be filled out by the independent auditor and transmitted via the internet to the LSC OIG after the audit is completed. On this page, you will find links to: the SRF Web-based application; the step-by-step instructions on how to complete and submit the SRF; and, for reference, the Legend of Noncompliance and Reportable Conditions Codes and a sample form. You are urged to print out and then manually complete the Sample SRF so that you will be prepared to enter the required data on-line. The LSC OIG will not accept the hard copy SRF, it must be submitted through this web site (not e-mail).
To run this application we recommend that your web browser be updated to the latest version.
Allow session cookie. The Summary Report Form input requires that your internet browser (e.g., Internet Explorer) accept a session cookie. To configure Internet Explorer 8 go to "Tools" then "Internet Options" then "Privacy" then "Sites." Add address of website: www.oig.lsc.gov click "Allow" then "OK" then "OK."
