Report a Birth Abroad
- Please read all of the instructions on applying for a Consular Report of Birth Abroad. Pay careful attention to the details regarding the required forms and proof of parental physical presence in the U.S. that is required. You will need to read the instructions closely to determine what original supporting documents you should present as evidence of parental physical presence.
- All clients must make an appointment through the online Appointment System. Appointments are available Sunday through Thursday, except for U.S. and Afghan holidays. You must print your appointment confirmation and bring it, along with a form of government-issued photo identification, to your appointment.
- At the time of your appointment, bring your CRBA application form, evidence of relationship between parent(s) and child, your statement entitled “Affidavit of Parentage, Physical Presence, and Support”, and sufficient evidence of parental physical presence in the U.S. Links to the forms and an explanation of parental physical presence in the U.S. can be found in the instructions on applying for a Consular Report of Birth Abroad.
