Eligibility Documentation
All eligibility documentation must be original or certified copies. A certified copy is a copy of a document or record signed and certified as a true copy by the officer to whose custody the original is entrusted.

Identification (ID)
All identification must be original. All customers ages 21 and older must provide two forms of ID; one being an unexpired state or government issued photo ID.

Lost/Stolen Common Access Card (CAC)
All customers requiring services for a lost/stolen CAC are required to present documentation from the local security office or CAC sponsor confirming that the CAC has been reported lost or stolen.

Sponsor Responsibilities
Sponsors have the responsibility to report within 30 days any family member additions or changes to the Defense Eligibility Enrollment and Reporting System (DEERS).

DEERS Enrollment (Spouse)
When adding a spouse to DEERS, the sponsor must present the following documents for their spouse: state certified marriage certificate, birth certificate, social security card, and a valid photo ID.

DEERS Enrollment (Child)
The sponsor must present the birth certificate or hospital proof of birth for a newborn child. The social security card will be required once the original card is received.

DEERS Enrollment (Child out-of-wedlock)
Child born out-of-wedlock to a male sponsor; must present the child’s birth certificate, social security card, and a court order establishing paternity or a Voluntary Acknowledgment of Paternity registered in the state of the child’s birth.

DEERS Enrollment (Step-child)
Adding a step-child; must present the parent’s marriage certificate, the child’s birth certificate, and social security card.

Dependent ID Cards
Dependent DEERS transactions/ID card issuance requires one of the following: presence of the sponsor; DD Form 1172-2 signed by sponsor in the presence of a VO/SSM; notarized DD Form 1172-2; or a valid power of attorney.

Full Time College Students (Age 21-23)
Students must present a letter from their school registrar stating they are enrolled full-time in an accredited institution of higher learning leading to an Associate’s Degree or higher.

The sponsor must indicate that he/she is providing over 50% support for the child in the remarks section of the DD Form 1172-2.

Cancel/Reschedule Appointment
Please keep a copy of your appointment confirmation page. Customers will need to know their confirmation number to cancel/reschedule an appointment.

Be Prepared for Your Appointment
Be prepared with valid eligibility documentation and identification. If you are more than five minutes late for your appointment you will need to reschedule or be seen as a walk-in customer.

Walk-in Customers
Walk-in customers are seen on a first come, first serve basis. Walk-in hours are dependent upon the number of customer waiting for services. Wait times for walk-ins can be up to 2 hours.
