APPOINTMENTS REQUIRED FOR ID Cards-DEERS

Online Appointments can be made for Family Members, Retirees, DOD contractors/civilians (CAC);
and Disabled Veterans. Appointments are not available for Soldiers at this location.
Soldiers are serviced at their unit or MPC, Bldg 130.

Appointments/Cancellations must be made 24 hours in advance to be processed.
One Appointment must be made per customer

REQUIRED: SPONSOR MUST BE PRESENT FOR ALL TRANSACTIONS OR DEPENDENT MUST HAVE
VERIFIED DD FORM 1172 (APPLICATION FOR ID CARDS/DEERS ENROLLMENT) CURRENT AND VALID
GENERAL OR SPECIAL POWER OF ATTORNEY (SPECIAL POWER OF ATTORNEY MUST STATE ID
CARDS/DEERS ENROLLMENTS)

Homeland Security Presidential Directive-12 requires two forms of identification to process ID cards for all adults.
Please ensure the names of identification Match your DEERS record

For ALL Lost or Stolen ID Cards a Ft. Hood Form 7006 must accompany your 2 forms of IDs before replacement.  The form can be obtained by clicking the link 7006. For CACs all Security Managers must SIGN form prior to issuance.  Managers must have a current signature card DD Form 577 on file at the Copeland Center Id Card Section.

Removal of SSNs from DOD ID Cards

SSNs will no longer be printed on ID cards and should not be relied on as a unique ID number. These changes
will occur when the cardholders' expired cards are renewed. The removal of SSNs will occur in three phases, all
of which will occur upon ID card renewal.
