Registration Information

Quest Anniversary Package
Includes the Quest Conference and the 25th Anniversary Gala. Advance registration $1275 and due by March 11, 2013. Registration after this date is $1425. See the registration form for complete information. Group, Faculty, and Examiner Discounts not applicable to the Anniversary Package rate.

Quest Conference Registration Fees
Includes breakfast daily, Monday am Award Ceremony and Brunch, Tuesday lunch.

Advance registration $1150 and due by March 11, 2013. Registration after this date is $1300. See the registration form for complete information. This does not include the 25th Anniversary Gala. One day options also available.

Group, Faculty, and Examiner Discounts for Quest Conference Rate Only
(Discounts not applicable to Anniversary Package rate)

Groups of 10 or more receive a $100 discount per registrant off the advance or regular Quest conference registration fee. Registrations must be made as a group. To register a group, select Individual/Group Registration from the drop down, enter the first registrant, and before checkout you will be asked if you wish to register additional attendees. Once you enter the tenth registrant, the discounted fees will be applied. The full conference fee will be charged if cancellations reduce the group to fewer than 10 people. Substitutions may be made at any time.

Full-time academic employees are eligible for discounted fees of $800 (advance) or $950 (regular). A $100 discount is also available to members of the 2012 National Baldrige Program Board of Examiners. The group, faculty and Examiner discounts cannot be combined with any other discounts and are not available for the Quest Anniversary Package.

To register at the discounted rates, select Group of 10+, Examiner, or Education Attendee from the drop down on the Registration landing page. Groups, Faculty, and Examiners may still register for both the conference and the Gala at a lower rate than the Anniversary Package rate by registering for the Quest Conference separately at the discounted rate and adding the Gala as a separate event.

Gala Options
Individual registration includes the Sunday Gala only. Advance registration $175; regular registration $200.

Reserved tables (seats up to 10) are available for a premium of $250 and must be reserved by March 11, 2013. The $250 premium does not include Gala registrations. Each individual seated at the table must be separately registered for the Gala at the applicable fee. The Quest conference will assign reserved tables and provide the location of the table on site on April 7 to the individual who reserved the table. This individual is responsible for inviting attendees to sit at the table. The standard cancellation policy applies to reserved tables.

Other than reserved tables, the Gala is open seating.

Pre-Conference Workshops presented by Award recipients
Sunday, April 7, 2013 • 1:00-4:00 pm
Limited seats available. Reserve early. Workshops are not a pre-requisite for attending the conference.
Workshop registration is $150. Does not include attendance at the Sunday Gala or Conference. Select either

Payment Information
Payment is due with all registrations. You will have the option to pay by credit card, check, or purchase order. Note: ASQ/American Society for Quality-E-Commerce will be shown on the credit card statement. Complete information is found on the payment section of the online registration form. Make checks payable to Quest for Excellence-ASQ. Checks or purchase orders should be sent to ASQ Customer Care, PO Box 3005, Milwaukee, WI, 53203. Checks must be accompanied by a copy of the registration confirmation to ensure payment is credited. You will receive confirmation of your registrations by email.

Cancellation and Substitution Policy
Requests for cancellation received on or before March 11, 2013 will receive a full refund. Requests received between March 12, 2013 and March 29, 2013 will incur a $200 processing fee. No requests for reimbursement for cancellations will be accepted after March 29, 2013. Substitutions may be made at any time.
