NY Connects
NY Connects is a program that provides information, assistance and referrals regarding chronic and long term care services and options. It serves any person of any age with a disability, chronic health issue or long term care need regardless of payer source. NY Connects services minimize the confusion of working through the long term care system to find comprehensive, objective information and support for individuals and their caregiver(s).
Individuals are assisted through a screening process to discover and discuss all possible needs and concerns. Following the screening process, the consumer can be provided with information on availability of services, assistance with benefit applications or making phone calls and referrals to sources providing specific services. The process guides the consumer in making choices but does not make determinations on behalf of the consumer. Primarily, it enhances consumer choice.
