ITS Hardware/Software Approval Form

• Complete this form to request IT approval for the purchase of hardware or software costing over $1000.00.  Please route this form to the Helpdesk by email to helpdesk@unthsc.edu, fax to 817.735.0266, or in person to LIB 205.
