Health and wellness, prevention and early intervention
We provide information and education on understanding and living with disabilities, connecting to wellness programs, and staying healthy and independent.
The ADRC offers information on programs to review medications, nutrition, teach people how to manage chronic conditions like diabetes or heart disease, eliminate home hazards, and prevent falls.
Health Promotion
Health Promotion includes the provision of planned activities to maintain or improve physical or mental well-being, accomplished through personal or collective behavioral and/or environmental change.
Nutrition Program
The program provides breakfast, lunch and/or dinner to people aged 60 and older, or 55 and older in tribal programs, with to those with greatest social and/or economic needs. Meals and other nutrition services are offered in a variety of settings, such as senior centers and community centers, as well as delivered to individual homes.
- Congregate meal programs give seniors the opportunity to socialize with others.
- Home-delivered meals are provided to frail elders who are unable to make their own meals, or for whom there is no one else available to do so.
- Persons age 60 and older who are homebound by reason of illness or disability, or otherwise isolated are given priority in receiving home delivered meals.
- Spouses of persons receiving home delivered meals, regardless of age or condition, may receive home delivered meals, when approved by area agencies, if such meals are in the best interest of the homebound persons.
- Nutrition screening, assessment, education and counseling are provided.
