Did you know that the Frontier Medical Home (FMH) has opened its doors to Active Duty Family Members as well as Retirees and their Family Members? The FMH is a Community-Based Primary Care Clinic, part of the Army’s initiative to improve primary care access and quality by locating clinics off-post in communities where Army Families reside. The clinic is an extension of primary care at Reynolds Army Community Hospital.

The clinic is located in a quiet and serene area of western Lawton and offers easy and quick access to healthcare. It is located across from Southwestern Medical Center, in the Great Plains Surgery Center building. All equipment and furnishings are brand new and state-of-the-art. The clinic also offers pharmacy, laboratory and behavioral health services. Most importantly, you will find a highly qualified and specialized staff that is eager to become your new healthcare team.

FMH is taking the lead in transitioning to the new Patient-Centered Medical Home model (PCMH). In this model, the Medical Home develops a comprehensive care plan as soon as the patient enrolls and then proactively engages the patient as a partner in their care. A team of health professionals, coordinated by a physician, works collaboratively to provide high levels of care, access and communication with its patients. At its heart, the PCMH is healthcare the way it should be - easily accessible, patient-centered, healthcare provider team-based, and quality-focused.

Active Duty Family Members and Retirees living in the Lawton area are welcome to enroll at the FMH either by calling 1-800-444-5445, going online to www.humana-military.com, click on ‘Enrollment’ and ‘Change PCM’ or come into the TRICARE Service Center located near the south entrance of the hospital for further assistance.
