Assessment

                                                                What is an Assessment?

An assessment is the first step in the process for Lehigh County residents to get the assistance they need.   Any of the providers listed below are able to complete an assessment. After contacting a provider, an appointment for the assessment is scheduled. During the appointment an assessor will gather information to determine the level of treatment that is needed.   In addition, assistance will be provided in completing the necessary paperwork to access treatment including a liability determination, release(s) of information, and may also include an application to the Department of Public Welfare for Medical Assistance. The assessment process may take up to 2 hours to complete.   Upon completion of the paperwork and assessment process, the assessor will determine the recommended level of care needed and provide assistance with the referral process.
