Application

Thank you for considering application to New Mexico Task Force 1 (NM-TF1).  New Mexico Task Force 1, the State of New Mexico, FEMA Urban Search & Rescue team (US&R) is currently accepting applications.  Applicants who wish to apply for any position on the Task Force must submit a completed application packet.  The application forms listed below are PDF fill-able forms and you can type directly to the form before printing.  Please save a copy of your completed forms for your personal records.  If you are able to visit the office please call for an appointment so that the Notary will be available.

 

All forms must be filled out completely.  Please pay attention to SPECIAL INSTRUCTIONS for some forms in the application.  Turn in application in person (by appointment, call Darlene for appointment, 505-383-9702) or mail completed and notarized forms to 5880 Office Blvd., NE, Albuquerque, NM 87109-5819 .  Be sure to bring your Driver's License and Social Security card with you when having forms notarized.  PLEASE NOTE:The US Appointment Affidavits, SF61 and DHS&EM Authorization for Release of Records forms must be Notarized.
